 After 19 years of serving the Brazos Valley, we have decided to close the clinic.

The last day that we will see patients will be April 27th, 2024. We will continue to provide medical care for you prior to this closing date. Please complete and return to our office an authorization form for you to  have your medical records forwarded to a new health care provider of your choice.

Once the clinic is closed, your medical records will be stored at Morgan Records Management. Beginning June 1st, 2024, future medical record requests can be made at https://morganrecordsmanagement.com or by calling 1-800-604-3994.  Prior to June 1st, 2024, call the clinic between 8am and 5pm Monday – Friday to request your medical records. Requests will be filled as per time limitations dictated by state law retention requirements.
